under $249.99	=	1 month
$250 or more but less than $1000	=	3 months
$1000 or more but less than $3000	=	6 months
$3000 or more but less than $5000	=	9 months
$5000 or more	=	12 months

For an applicant, the penalty commences the month of application
For a recipient, the penalty commences the first month after expiration of the required notice period
